Ballistix memory gone bad

On 8/13/2009 I purchased two 4GB (2x2GB) kits of Ballistix DDR 800 RAM from newegg.com. Here is the actual item on newegg's site: http://www.newegg.com/Product/Product.aspx?Item=N8
1 of the 4 sticks has can bad. I have isolated it by testing each stick of RAM individual with Memtest86+ v4.0, allowing 100% of the tests to run for 1 full pass on each DIMM individually.
The DIMM that fails has over 125 errors when the test is complete.
The information on the DIMM itself:
CL111B3.BC 103566
BL25664AA80A.16FE5, LEAD - FREE w/EPP
800Mhz 4-4-4-12 2.0V
How do I go about getting the single DIMM replaced?

You will need to contact Crucial Customer Support and return the whole kit. They will send you a new matched pair.
